Western Medical Associates’ policies are as follows:

No-Show / Cancellation

It is the policy of all Western Medical Providers to encourage patients to arrive and receive care at their scheduled arrival time or to give appropriate notice of cancellation to allow other patients to receive timely care.

If you are unable to make your scheduled arrival time, we request that you notify us as soon as possible, but no later than 24 hours prior to your scheduled arrival time.  Additionally, we request that you arrive at your scheduled arrival time.

By either not providing 24 hour notice or arriving late, you may be assessed a $40.00 fee for a missed office visit or $50.00 fee for any missed procedures.  If three (3) or more appointments are missed, Western Medical Associates reserves the right to terminate our relationship with you.

It is not our intent to assess an additional financial burden, but it is costly if you miss your appointment and do not give us time to schedule another patient in you time slot.

Insurance

We participate in most insurance plans, including Medicare.  If you are insured by a plan we do business with, but don’t have an up-to-date insurance card, payment in full for each visit is required until we can verify your coverage.  Knowing your insurance benefits is your responsibility.  Please contact your insurance company with any questions you may have regarding your coverage.

Self-Pay

If you do not have insurance or are receiving a non-covered service, it is our policy to collect a $100 deposit before treatment.  It is important to note, if the total cost of the visit is more than the deposit collected, an additional payment will be required after the bill for services is generated.  You will receive a statement within 30 days of service.

Co-payments

All co-payments must be paid at the time of service.  This arrangement is part of your contract with your insurance company.  Failure on our part to collect co-payments from patients can be considered fraud.  Please help us in upholding the law by paying your co-payment at each visit.  Failure to provide co-payment at the time of service can result in your appointment being rescheduled.

Proof of Insurance

All patients must complete our patient information form before seeing the provider.  We must obtain a copy of your driver’s license and current valid insurance to provide proof of insurance.  If you fail to provide us with the correct insurance information in a timely manner, you may be responsible for the balance of a claim.
